Guide Healthcare is committed to assisting older adults to remain at home through flexible and effective allied health care. Our clinicians empower and assist elderly home-dwellers to remain independent for longer. This may be through focusing on improving their ability to complete activities of daily living, maintaining mobility, providing pain management strategies, managing falls risk, coordinating home modifications, improving foot care or offering nutritional advice and support. With an existing network of experienced allied health staff, we can integrate into a person’s lifestyle and meet their care needs promptly.

How does it work?

Submitting a referral? Here’s a step-by-step of how the process works.

Step 1

Support Coordinator completes Referral Form and email to homecare@guidehealthcare.com.au

Step 2

Guide Healthcare to contact client to arrange visit and advise Support Coordinator.

Step 3

Clinician visits client and completes initial report and service agreement.

Step 4

Support Coordinator to confirm approval to proceed if necessary.

Step 5

Guide Healthcare/Clinician to confirm any follow-up visits with Support Coordinator.

About our Home Care Service

Guide Healthcare has partnered with home care and CHSP providers to assist older adults to continue feeling great at home through flexible and effective physiotherapy and occupational therapy services. We recognise that every individual is different, and this is reflected in our approach and treatment plans.

John’s Story
