  • Guide’s NDIS occupational therapy services help people living with disability build independence, safety, and confidence in everyday life. Our occupational therapists provide personalised support and strategies with daily activities, home and environmental modifications, assistive technology and equipment prescription, and strategies to overcome functional or cognitive challenges enabling participants to achieve their goals and live the life they choose.

Submitting a referral for NDIS? Here’s a step-by-step of how the process works.


Step 1 

Support Coordinator completes Referral Form and email


Step 2

Guide Healthcare to contact
client to arrange visit
and advise Support Coordinator.


Step 3

Clinician visits client
and completes initial report +and service agreement.


Step 4

Support Coordinator to confirm approval to proceed if necessary.


Step 5

Guide Healthcare to confirm
any follow-up visits
with Support Coordinator.
